1. The floor must be clean of all debris, plaster, dirt, etc.
2. The sub-floor must be leveled within 1/8" using Portland Cement-based underlayment.
3. The sub-floor must be vacuumed.
4. The slab must meet acceptable moisture range (8% or below).
5. Permanent lighting must be installed and working.
6. All other trades needing access to the floor area we are installing must be completely finished.
7. Adequate room must be available for storage and staging of materials other than the gym floor.
8. All non-gym flooring materials (gym equipment, fitness equipment, etc.) must be removed from the floor.
9. The installer must be contacted prior to commencement about any specific time requirements (start and stop) that must be adhered to.

It is understood that the above conditions must be met prior to Centaur installers' work starting.  Should Centaur installation crew arrive and be unable to start, your hereby accept the travel and labor cost for their remobilization.  Should the crew be required to perform any sub floor preparation or move equipment, you will accept responsibility for these charges as well.

If any of these requirements cannot be met, or are not understood, you will be forwarded to contact the installer.  This form must be signed and faxed to our Corporate Office prior to installation.
